  5. Short Term DisabilityDisability coverage replaces a portion of your income if you become totally disabled because of a covered accident or sickness. Key Features: On/off job or off job accident & illness coverage, includes partial disability benefit & waiver of premium benefit. • Plan Design: Up to $5,000/monthly income replacement not to exceed 60% of income. Benefit periods from 3, 6, 12 or 24 months. Elimination periods starting at 0/7. • Optional Riders: Wellness Benefit • Special Underwriting: PEGI & GI; Credit for Time Served • Offered To: Employee

  6. AccidentDesigned to help see you through the different stages of care in the event of a covered accident. Helps to offset out-of-pocket expenses associated with an accident. Key Features: On/off job accident coverage, emergency room, X-ray, ambulance, fractures & dislocations, surgical care, transportation & lodging, hospital admission & confinement, ICU admission & confinement, follow up care, AD&D, catastrophic accident • Plan Design: 3 Levels with or without Health Screening Benefit • Optional Riders: Employee Disability; Spouse Disability; Sickness Hospital Confinement • Special Underwriting: No underwriting on base plan. • Offered To: Employee, Spouse, Child(ren)

  7. Medical BridgeHelps pay for deductibles and co-insurance as well as every day living expenses. Key Features: Hospital confinement, rehabilitation unit benefit, outpatient surgery benefits. Some plans offer diagnostic procedures, ER visit, doctor’s office visit. Includes Wellness Benefit Plan Design: 5 Levels Special Underwriting: PEGI & GI Offered To: Employee, Spouse, Child(ren)

Key Features: Pays benefits for critical illnesses such as heart attack, stroke, major organ failure, end state renal failure, permanent paralysis, coma, blindness, occupational HIV or occupational hepatitis B, C, or D, and coronary bypass surgery. Plan Design: Subsequent Diagnosis with or without Wellness Screening Benefit Employee - $5,000 to $100,000 Spouse - $5,000 to $40,000 Child(ren) - $25% of EE amount Special Underwriting: Simplified Issue; Simplified Issue (level 1); PEGI Offered To: Employee, Spouse, Child(ren)
