  1. Entering Visit Records in ISPED How to use the new Visit Record - individually and with the Group Visit Record Feature

Why a new format? • due to a hearing settlement, additional data will be collected from related service providers • this data will protect both student and provider by providing clear documentation of services: • when provided as planned per the IEP or MP • when no service was provided due to student or provider absence • when alternate activity or alternate arrangement was provided due to provider absence

  3. What is the additional data? • for regular service • provider will document the primary method of contact/intervention method used by selecting from a drop down menu • if student not available • provider will document whether student is absent or at a school event

  4. What is the additional data continued… • if provider not available, provider will document one of the following: • no service provided and inform parent • alternate activity provided along with date of provision • alternate arrangement provided along with date of provision and actual provider if not regular provider

  5. Regular Visit • radio button for regular visit is the default selection • enter date, visit length, primary method of contact/intervention, and projected next visit date • enter Actual Provider if not regular provider

  6. Primary method of contact / intervention • select primary method of contact or intervention from dropdown • all related service providers will be entering this data

  7. Student Not Available • select radio button “Student not Available” • indicate why student was not available • enter date student not available and projected next visit date

  8. Provider Not Available • If provider is not available there are 3 possible scenarios to document: • No service provided • Alternate Activity provided • Alternate Arrangement made for service

  9. Provider Not Available – no service provided • select “Provider Not Available” • select from dropdown menu service “Not Provided” • select radio button to indicate if parents were informed

  10. Provider Not Available – Alternate Activity • select “Provider Not Available” • select “Alternate Activity” • enter date of alternate activity

  11. Provider Not Available – AlternateArrangement • select “Provider Not Available” • select “Alternate Arrangement” • enter date and minutes alternate arrangement • indicate primary method of contact or intervention & actual provider, if needed • 3 possible alternate arrangements available

  12. Visit Record Summary • new visit record summary format provides useful data • projected next visit date does not display when alternate activity or alternate arrangement is selected • no service provided is in RED text, to alert provider  it may be edited if service is later provided  text will turn black

  13. Using the Group Visit Record Feature • the group visit record feature can continue to be used to document: • REGULAR VISITS • STUDENT NOT AVAILABLE • the group visit record feature cannot be used yet to document Provider Not Available. This can only be entered on an individual Visit Record from within the Service module.

  22. SCENARIO 4 Services provided as usual on 4/19. You also inform the student & teachers that you willbe at a workshop on Thursday, 4/22, so you will come on Friday, 4/23, instead.

  26. SCENARIO 7 You got stuck in Vegas – -so you did not come on Monday, 4/26.

  28. SCENARIO 6 You have a flat tire on your way to work on 4/29. The next day, student completes a mini-project in the regular classroom that you assigned.

  30. SCENARIO 8 For your 4/26 absence, you provide the following: May 3 & 6: you added 10 extra minutes to your usual session May 7: another SLP provided your student services for 10 minutes

  33. GROUP Visit Record Now use the Group Visit Record feature for 3 of your students. Scenario 1: Group session on May 10 for all 3 of them for 45 minutes. Project next visit is May 13.

  35. SCENARIO 2 You saw the 3 at different times: Student A: individual session on May 12 for 45 minutes Student B: absent on May 13 Student C: observation by Minnie Mouse on May 14 for 30 minutes
